Do gallstones improve or worsen on a low carb / high fat diet? It’s a common question with an interesting answer.

The gallbladder stores bile, a yellow-green fluid manufactured in the liver. The bile is used to digest the fat you eat. The question is: Is it good or bad for the gall bladder to eat fat?

The conventional fat phobic answer

The usual medical belief today is that eating fatty foods can result in gall stones. This is because what happens if you already have gallstones in the gallbladder and eat fat: A gallstone can get stuck on the way to the intestines and give you a gallstone attack (pain in the top right part of your stomach).

The conventional advice is thus to eat low fat – and take pain killers if you get a gallstone attack. If the attacks continue the gallbladder is removed by surgery and the problem usually goes away. Probably with the side effect of slightly decreased ability to absorb fat and nutrients from what you eat (there is a reason we have gallbladders).

The conventional low fat advice rarely makes gallstone disease go away. Instead it often gets worse with time, until surgery is necessary. That is hardly a coincidence.

How to get gallstones

If you eat low fat less bile is needed to digest your food. More bile thus stays in the gallbladder. Long enough, perhaps, for stones to form. It’s been shown that people who (instead of fat) eat more carbohydrates are at increased risk of gallstones.

It all sounds logical. And there is even better evidence. The risk of low fat diets have been tested at least three times:

Studies of extreme low fat diets


  • In a study of 51 obese people using an extremely low fat low calorie diet (just one gram of fat a day!) the gallbladder was examined by ultrasound before the diet and after one and two months. After one month four of the 51 participants had developed new gallstones. After two months more than one in four (13 people) had new gallstones! This on an almost fat free diet. Three participants needed to have their gallbladder removed during the study.
  • A similar study examined 19 people eating an extremely low fat low calorie diet over 16 weeks. At the ultrasound examination at the end of the study five people (again about one in four) had new gallstones.
  • A third study compared an extremely low fat diet with a diet slightly higher in fat during 3 months. More than one in two (6 of 11 people) in the group eating extremely low fat developed new gallstones. Nobody in the group eating more fat did.

Conclusion: Avoiding fat increases your gallstone risk!

What happens if you do the opposite?

What if you were to do the opposite of the usual advice? Regularly eat food with fat in it? Then more bile will be used to digest the food. The bile ducts and the gallbladder will be flushed through regularly. Theoretically stones will not have time to form, and pre-existing stones might (if you are lucky) be flushed out into the small intestine.

The risk is that if you already have gallstones, you may get discomfort as you flush them out.

The question is: Do you want to think short-term (low fat) or long-term (higher fat) solution?

Does high fat food work?

It’s logical to think that food higher in fat can result in a gall bladder free from gallstones, and the science supports it. One randomized controlled trial compared a higher fat to a lower fat diet in obese subjects over the course of 6 months. 1 The Higher fat group had better gallbladder emptying and no stones developed, whereas over 50% of the low-fat group developed gallstones. This was despite weight loss in both groups.

This fits with clinical experience and anecdotal reports from those who have experienced their gallstone disease disappear on a LCHF diet. Sometimes at the expense of initial gallstone attacks though.

Gallstones and kidney stones

Let’s compare the advice we give patients with kidney stones to the advice for gallstones. We tell patients with kidney stones to drink a lot of fluid, increasing the production of urine, so that stones do not have time to develop. If you already have kidney stones this advice could give you a painful attack initially as you pass the stone. But doctors still advise this despite the short-term discomfort as it is the better longer term solution.

The reason we give the opposite advice when it comes to gallstones might be the misguided fear of fat. If we were afraid of water instead, patients with kidney stones might have been advised to avoid drinking to avoid kidney stone attacks. If they did not improve, would we surgically remove their kidneys?

Another common question is if you can eat LCHF if your gallbladder is already removed. The answer is that this seems to work fine.

Some people without a gallbladder might have to increase their intake of fat gradually to allow their body time to adapt. Otherwise the body might not have time to digest the fat which could result in loose fatty stools initially. However this rarely seems to be a problem.

Why would drinking more water cause a kidney stone attack - initially - and would then the advice to drink more be correct or just an example of what not to do? It seems as if drinking water IS good but I'm not entirely sure I have understood you.

    I had my gallbladder removed over 10 years ago and when I tried this diet, I had a lot of tummy upset /pain/gas/diarrhoea which seemed to be related to the amount of fat in the diet and the reduction of bile, sans gallbladder, to break it down. Any suggestions to a solution for eating LCHF without a gallbladder would be much appreciated as I am borderline Type 2 diabetes and need to get my blood sugar level down. Thank you.

    Marguerite, it is 8 years since I did my gallbladder flush, so the same recipe took a bit of finding. I found it at: under the heading; Epsom Salt and Olive Oil Detox. It is practically verbatim of the one I used.

    A friend tried another recipe, but it didn't work for her. Mine was far more detailed, though, with several days preparation.

    I feel obliged to say that I am not a doctor and I don't know your medical history. Having your doctor's advice and support would be reassuring. However, as intelligent adults we have to make our own, informed, decisions, at the end of the day!

    When you check out the recipe you will see you have a choice of lemon or grapefruit; if it is of any interest, I used the latter. If you decide to do it, I would love to know the outcome. Following are some tips: 1. Drinking the final concoction through a straw, is a good one. 2. Time it in advance to suit your schedule, and make sure you can have a complete day of rest, the day after the flush/detox.

    My experience: No sooner my head touched the pillow than I had to throw back the covers and run to the bathroom, where I seem to remember spending most of the night. So be prepared! All the best,

    Hello Animor, going by your last paragraph you seem to have got it, at least as I understood it! It was simply an analogy to highlight the irony of the standard advice. Perhaps the good doctor would correct me, if I am wrong.

    The standard advice does leave those practicing it with "their knickers in a twist," when it comes to seeing the logical answers on this and other issues, like diabetes!

    As water flushes the kidneys, oil keeps the gallbladder flushed and refreshed. However, when there are already stones, it is can be quite a painful process. There again it is very painful having the stones and there is always the chance that a gallstone will block the exit, requiring immediate surgery, which can happen at any time. Stones are better out than in and for myself I preferred the flush to the surgery. Though, of course, it cannot be guaranteed to work! But, there again, nothing ventured, etc.

    I started having gallbladder problems after I did a very low fat diet. I have been in and out of the hospital 5 times where they had to give me morphine to take away the pain. I was lucky as my Dr had an Ultrasound done and we found out that my stones are about 2 cms. I had done the cleanse a 3 weeks ago and got a few out. I am now going to do more until I get no stones coming out. Amazingly after I did the cleanse the pressure in my right side was gone. Although it has since come back I understand that when you do the cleanse that sometimes you release the stones in the front of your gallbladder and then the rear ones move forward.
    It was much easier than I thought. But the Epsom Salts were horrible!!! The grapefruit and oil mixture was really quite tasty. I thought that part would be quite unpalatable. I put the mix into my Magic Bullet and voila it was all mixed and ready to drink.
    To avoid any problems just make sure you know before that you don't have any big stones or you could land in the hospital and have to have your gallbladder removed. Then take the time to prepare for the 5 days before. It is well worth it (except for the horrible Epsom Salts!).
    You can look at the Livestrong method or Hulda Clark's cleanse as well.

    I have some questions! I did a gallbladder flush about 6 months ago after years of food allergies and tummy pain. The results were amazing and for the first time I was able to tolerate eggs, avocado, banana and other foods. Since then I've been following a lchf diet and loving it, but the last 2 days I've started getting gallbladder pains again particularly after breakfast (bacon, eggs and avo). After reading everyone's comments the general consensus seems to be that more fat is better for gallbladder health, but in my case it seems the increase in fat is triggering pain again. I would really appreciate any advice!

    Hi everyone, I had my first attack when pregnant with my first child 16 years ago, although I didn't know it was gallstones at the time, I went for years tolerating the pain (approx. once per month) until 8 years ago the attack was sooooooo bad I ended up in hospital to be told it was gallstones and they were blocking my liver (the whites of my eyes were yellowing), they removed the stones orally under sedation. I have been fine for years up until about 6 months ago when I went on the LCHP diet, I found that yogurt was what irritated my stomach to a point were I was physically sick, since then I have found a fantastic pain relief APPLE CIDER VINEGAR, a splash of this in undiluted juice each morning and just drink in one. This is said to dissolve stones and 'touch wood' ive not had an attack since January - 2 weeks after starting the apple cider vinegar.

    Yes, a 'splash of apple cider vinegar' in a 'splash' of undiluted juice, not a full glass, I assumed I was talking to intelligent people therefore, didn't feel the need to write a splash of undiluted juice also, I thought this would be obvious, my mistake.

    I also had my gallbladder out. Mine was taken out in 2009. I'm also pushing the envelope of type II. Now when I try to do Low Carb, I get some major stomach pain right below my rib cage/upper stomach. It's almost exactly where I used to have gallbladder attacks!!! Once I had to go the emergency room, because the symptoms were that of a gallbladder attack. The less than helpful staff, couldn't say what the problem was. I am wondering if there is a gallstone in a bile duct. Even if so, why does only eating low carb produce these symptoms.
    That was a year and a half ago. Now I am trying low carb again, and on my second day, I've started getting a little bit of that upper stomach pain AGAIN!!! Well I just went and made a sandwich, and it feels a little bit better. WHY DOES THIS HAPPEN?

    I was on the HFLC diet for about a year when I developed bad gallbladder pains. I scanned positive with a 13mm gallstone and advised there was no way I could avoid having my gallbladder removed. Petrified I would not be able to eat high-fat low carb again I can confirm I am positively able to be back on this diet.

    What I am unable to do is eat wheat. That brings me severe diarrhea. I suggest anyone who has had their gallbladder out who is thinking their diarrhea is because they are eating fat with carbs, following gallbladder surgery should remove all carbs and see what happens. You have nothing to lose.

    I now realise the pains I use to get about 12 years ago must have been gallstones, may be they passed, may be they lay dormant, but this is when I was doing raw foods, juicing and low fat. I now wonder if the stones became bigger when on the HFLC diet because by going into Ketosis I was losing lots of weight and with it lowering my cholesterol, which has to surely be passing through the liver and gallbladder.

    This is an absolutely ridiculous article! There is not a person on earth who can benefit from a high fat low carb diet! I was on a high fat low carb diet most of my life. (Unintentionally) Both my parents were diabetic so I was just raised that way. Despite my "healthy" lifestyle I gained weight and started to have gallbladder pain, and pre diabetes! Well I recently went to a high carb low fat diet, and gues what? I completely reversed my high ac1 count, stopped having gall bladder pain, have lost weight and continue to slowly lose it. This article is going to kill people! You should never be on a diet that excludes all fats! But you shouldn't promote a diet that says don't eat carbs and eat all the fat you want! That's insane. Carbohydrates are so important to our health and well being. Again this is a terrible article and I pray that people who read this consider other options before clogging their arteries and causing themselves to become insulin resistant.

    My well admired Seattle cardiologist even recommends a LCHF diet. He doesn't support eggs and bacon all day, but lots of vegetables, adequate protein and lots of healthy fats, which are loaded with nutrients. You will deminish health problems and stabilize blood sugar if you follow this protocol. Apple cider vinegar or ACV is a tonic for your liver and gallbladder. Malic acid in the apples dissolves cholesterol formed stones and Is a godsend during an attack. I am starting the magnesium/Epsom flush, but prepping a few days before with vegetable soup and ACV since I cannot have any juice and it better for overall health. Whenever I have gallbladder discomfort I drink 2 Tbl. Raw ACV with 8 oz. filtered water and stevia drops, my symptoms cease every time. As soon as I went off of my LCHF diet, my gallbladder discomfort has gotten worse, which is why I am doing the flush. Sticking with any restrictive diet is next to impossible long-term, hense the typical yo-yo diet frenzy! The fair scones at the Evergreen Fair are tough to resist.     Healthy, active, 35 year old woman doing LCHF for 3 years. Most recent tests revealed polyps on my gallbladder. I've read that these can just be fat accumulated. I will retest in a year to see if they're still there or disappeared. Anyone any idea?

    it could be because you may be taking too many calories, in form of fat or carbs. reduce the calories, but keep high fat and low carb diet and see how it goes.

    I have done over 50 liver and gallblader flushes after 40 I didnt get many stones but I do 2 a year to maintain. I did it over 13 years as I was internally feeling very ill. MY Husband bought me a book. In the book was Hulda Clarks liver flush recipe it can be found on Cure zone website. There are lots of recipes but I've found this to be the best There are also lots of pics. Very useful website. I love it.
    I have to say it's been incredible! Really. The first few flushes were amazing. The stones the next day we're so many. 2 1/2 plastic cups full. They all float on the top of the water. You can scoop them up using plastic spoon and plastic cups. Just so you can take a look. Take pics. I'm sorry I never kept some of mine. Dose help others to see them on curezone. Some where calcified and some large green and hard. And then there's lots of chaff like rice. But the feeling I had after they are passed and rested for a time. Is fantastic. So much energy. And peace and rest in my abdomin. Hadn't felt that way since I were young. I would do these flushes with my husband. Which is great. Bit of company and compare stones etc. Strange way to spend your weekend yes but well worth it. Family thought us mad but no we just normal people looking out for a better way to live. They laugh at us in China and Canada and parts of US getting our galbladder out! We're it is used more.
    I was passed from pillar to post of consultants. And they all couldn't help me. I was so clogged up eating the western diet! So I needed to function as I'd not long had my lovely son Alex. So I took my health into my own hands and have not regreted it one little bit.
    Please note. We did a Parasite icleanse and oxy powder cleanse first for preparation. So the stones can flush out easy. Epson salts open up the bile ducts. Which is in the recipe. Read it all before you start.

    Stones only flush out easily if small enough to fit through the gall bladder duct. If they are too large you get blockage, extreme pain, possibly pancreatitis. Same eating high fat diet with stones. Likely good preventive but a real dice roll if you already have them!

    I don't believe for a second this was written by a doctor.

    I have two friends who had gallstone problems due to high fat diets (one even followed the Paleo Diet for years).

    Both have had their surgery to remove their gall bladder, and now they can't eat fatty food at all without needing to take a dump shortly thereafter. They simply can't eat high amounts of fat anymore, their body can't digest it.

    This is why people need to get off the Paleo Diet and similar high fat diets. It's bad for you.

    I found sticking to unsaturated fats is fine ie. bacon, butter and the fat off lamb is a no go but olive oil and lean meats are fine. Weirdly chicken skin doesn't seem to be a problem.

    Elizabeth, low carb is not no carb. Many fruit and vegetables have carbs in so just cutting out grains may be enough for many people. Or just eating small portions of carbs.

    I always followed low fat high carb and got gallstones ..... I had to have my gallbladder removed as there was no way to get the blighters out!?

    I went on the low carb diet and after a couple of weeks had a terrible attack of gallstones. I didn't know I had them before this. Ended up in A&E on mega pain killers. Had a scan and found I have two large gallstones. I'm afraid of going low carb now. The pain was awful, I have had no attacks since.

    Like others have said, everyone's body is different. I know there are foods - for example eggs, bacon, and sausage - that always cause gallbladder attacks for me. But fats like avocado, seeds, nuts, grapeseed oil, coconut oil, and even some cheeses don't seem to trigger gallbladder pain at all.

    Eating low carb with lots of the healthy fats I mentioned above works well for me - but if I start consuming too many animal products, I seem to have more issues.

    Also, in addition to the Apple Cider Vinegar, I also have found that 1 teaspoon of ground flaxseed mixed in a 1/2 cup of warm water stops a gallbladder attached dead in its tracks.

    I did liver cleanses for nine years, took gold coin grass (sensible which saved me from many painful attacks, but eventually needed to have my gallbladder removed. I was told by my doctor that I could return to my normal way of eating. Not! I had done some research on what happens when people have their gallbladder removed and found that many die of heart attacks, presumably because of fat in the bloodstream since bile was no longer available in the same amount. I found that some food went right through me and thought I would spend the rest of my life making sure I was near a bathroom because diarrhea was going to be normal for me from now on.

    Fortunately I had a live blood test done at my naturopathic doctor's office and I could see the fat molecules floating around in my bloodstream. Very unnerving. I was told I would need to take digestive enzymes for the rest of my life so that this would be controlled. I began taking the enzymes, and subsequent live blood tests have shown a marked decrease in the amount of fat in my bloodstream. Plus I don't have to worry about loose stool as long as I take the enzymes. I hope those of you who had had your gallbladder removed will take a serious look at digestive enzymes, especially if you are on the LCHF regime. It will definitely make a difference.

    I went on a very low calorie diet in 2012. Lost lots of weight. Got gallstones, visible on ultrasound; a couple of attacks, I refused to have my gallbladder removed, and symptoms faded in time. Six months or so ago, I started following LCHF (which my doctor recommended), and recently had another nasty attack. I had another ultrasound this morning to check the status of the previously recorded gallstones this morning... and they're gone.

    I guess the fat intake flushed the stones steadily, as the advice on water flushing kidney stones referenced above suggested it might.

    Either way, I don't care - officially gallstone free!

    Having no gallbladder as mine was completely solidified and removed, it was an interesting transition. First, it should be stated that I was also diagnosed with IBS. So, the general population surrounding me including my doctor told me that I would be in for some real problems with the LCHF diet. I did for about 5 days. I am very happy to report that with my transition to LCHF there are less issues than before. In fact my IBS is gone! On of the common side effects to no gallbladder is the unprecedented need to instant washroom requirements. Add IBS to this and life is not a lot of fun.

    Moving forward, after the initial five days, my body began to regulate and all issues have subsided. That said, if I am really enjoying fatty foods and perhaps take in more fat than my body is use to at a meal there is a small amount of discomfort but its got in 10 minutes or so after eating - not sure what it is but compared to my past life it is nothing and happens maybe twice a month.

    Should I be "bad" and eat too many carbs oh boy, just like yesteryear with several cramps, bloating, diarea and generally feeling like S@#$. Yep watch out for that fat!! It is so evil it even assists in this department (sorry sarcasm).

    Just over a year (seriously) into LCHF and the benefits are so overwhelming as it is really hard to look back and state that the food you were eating has caused 90% of your health issues that you had. Biggest advise I can give is stick with it! Give your body a chance to redefine what NORMAL is for you...its worth it!! My 6 year old grandson has even noticed the difference as Grandpa is able to play more!! If that is not proof nothing is.

    To close - less issues on LCHF than not with gallbladder issues, yes it may take time but it is worth it!!

    Grateful for this post, I did start having pain in upper right part of stomach (after month of LCHF). I have no history of gallbladder problems/stones (but have not done a check for 3 years). I also thought that gallbladder is on the left. This pain was present for 4-5 days, but not very intensive, and usually only if I would bend down or stand up. I had a hard time with digestion during those days (unlike the rest of the diet so far) and also felt a bit nauseous when using the loo. But then I had this flushing out of the system, and have not felt any pain in the last two days at all. I do drink a lot of liquids, but in following the LCHF diet I usually have coffee with little milk for breakfast, proper LCHF lunch, and in evening some mature cheese and salad, but not a full LCHF meal for dinner.

    Tried one of the Atkins frozen meals. Triggered a gallbladder attack that sent me to the ER. I don't think I would trust everything you read on the Internet. So much for the Atkins LCHF approach.

    I have had gallstones and attacks for 7 years. I have done 8 or so of the Hulda Clark liver flushes. They always helped relieve my symptoms for a time. I did 2 flushes 2 weeks apart and ended up getting a gallstone lodged in a very painful place. It took one week to dislodge (and a whole lot of pain and a liquid diet only). I did not want to have my gallbladder removed because I firmly believed God put it there for a purpose so I stuck it out through all this pain. Then, I got pregnant unexpectedly and was terrified I'd have more attacks while pregnant. My doctor tried to convince me that I should have my gallbladder removed while in the 2nd trimester. I had no peace about doing this surgery while pregnant and cried out to God for answers. Through some research, I learned that components in the bile can get out of balance. I wondered if my choline was low due to the fact that I learned I had an egg allergy in 2005 and quit eating them. By 2009 I had my first gallbladder attack a few weeks after my 5th child. Now, that I was pregnant with my 6th, I needed some answers. I started taking sunflower lecithin in capsules because of its choline content. When any minor gallbladder symptoms started I would increase the amount for a few days and the pain would go away. I also faithfully used digestive enzymes with every large meal (usually lunch and dinner). My enzymes had ox bile extract as well as other enzymes to break down fats and proteins. I also started using an herbal combination for liver support. These 3 supplements have been my answer. I'm so grateful God gave me answers when I cried out to him! My 6th child is now 2 and I have had no gallbladder attacks and real pain or issues through the whole 2 years, even with the fluctuating hormone levels and the postpartum weight loss.

    I'm still a bit of a beginner at low carb but not a beginner at gallstone pain! I will reinforce the apple cider vinegar relief but please make sure it's diluted in water if you want to keep your teeth and voice!

    In terms of trigger foods, I seem to be the opposite from others in that eggs, bacon etc don't give me pain but avocado is a major trigger food and I've recently found coconut oil is too. I think the low carbing (the high fat bit) needs to be taken in stages and built up, gallstone grumbles may be ok but full on attacks are incredibly painful.

    I guess its the simple fact that what may work for one person may cause the opposite for another.
    I am day 4 of the Keto Diet. Im taking the keto/os powder each day, which puts me into ketosis within 30-60 minutes after ingesting it.
    I found out I had gallstones about 4 years ago. Luckily ive never had an attack, but did experience many symptoms. At first I thought it was from fatty foods, but I was wrong, it was from anything with wheat.
    I know its only day 4, but ive cut out all carbs, upped my fat intake, eating moderate protein and ive not had to reach for my antacids once.

    I currently have a 1cmx 1cm x 1cm gallstone (which is funny due to the fact that my gallbladder is only 4-7 mm in size- so my stone is larger than the organ). The doctor told me that the bigger ones usually aren't issues, as they won't go anywhere and cause infections. I started this diet and noticed gallbladder attacks at times... but I'm dealing with it... not severe enough for surgery.

    Question I don't have gallbladder stones, but my gallbladder doesn't contract . At least this is what I was told a few years back. Have you ever heard of this how do I lose weight if iv been told to stay away from fats ?

    I had my gallbladder removed about 7 years ago. At that time, my surgeon and other specialists all said the same thing, "most times gall stones are caused by not drinking enough water".
    When I thought about it, it's true. The jobs I was doing did not allow for me to drink as much water as one should. If you are overweight, you have more blood and need to drink more than the required 8 glasses per day. No matter how hard I tried, I never got my 8 glasses of water in a day, and still don't. Also, I agree with stay away from the wheat, it has 424 proteins now compared to 4 that it originally had before we decided to modify it. Our bodies can not break these proteins down.
    High fat, low fat-everyone is from a different ancestry and blood type. Some people do better on high protein as others do not. My husband sits down and eats a 12 oz steak and then looks for more food. All his blood work is normal and he is 46. As for me, I can eat a small drumstick and done for meat. You need fat in your diet to make hormones. Our daily living is bombarded with lots of carbs throughout the day. Have a training session at work and count how many carbs are offered and why you feel tired at the end of the day. Morning snack: Muffins, fruit, yogurt, bagels(equals 6 slices of bread) or cereal. Lunch: Pizza or subs. Afternoon snack: cookies and fruit.
    When I reduce my carbs, my glucose levels go down and my weight goes down. Those of you getting stomach aches from eating the fat, its probably the cheese and you may be lactose intolerant.

    I heard ox bile (it's a pill) taken with high fatty meals will act as sort of a gallbladder and assist with your body breaking down the fats and better absorbing. I am going to try this as this diet, while I love it - is giving me a fit with my stomach and intestines. Btw - I had my gallbladder removed as well.

    Finally I've run into another person who can't eat avocado! It took me so long to figure out what it was, because I always ate guacamole when eating Mexican food, and always thought it was an unusual spice. They gave me gall-bladder like attacks, and now I know it really is that.

    Because increase in water intake will basically flush the kidney stone out. When the kidney stone moves it causes pain. So despite the increase in pain from a kidney stone moving out of the kidney, along the ureter, in the bladder, and along the ureter out of the body - increase in water consumption is still encouraged.
